Care Choices HMO

Available through employers in southeast Michigan.

Care Choices HMO is a nationally accredited health plan that features comprehensive coverage for a wide range of preventive and medical services, including routine office visits, well-baby care, immunizations, hospitalization and worldwide emergency care. Prescription drug coverage may also be included in some benefits packages.

Members choose a primary care physician from a network of participating providers. The PCP will provide or arrange care for all of the member’s medical needs. In the traditional HMO, services are typically covered in full with copayments, which helps keep out-of-pocket costs and paperwork to a minimum. Care Choices HMO also offers consumer-directed plan options that are subject to deductibles and coinsurance.

Care Choices HMO has been rated #1 in southeast Michigan for member satisfaction and #1 in Michigan for clinical excellence by the National Committee for Quality Assurance.
