Care Choices HMO

Care Choices HMO is a nonprofit managed health care plan that holds the highest level of accreditation, “Excellent,” from the National Committee for Quality Assurance (NCQA). A recognized leader in clinical and service excellence, Care Choices HMO received the highest rating in all five categories of the 2006 NCQA report card. Care Choices HMO was also recognized as the top-ranked health plan in Michigan and #12 in the nation according to the U.S. News & World Report /NCQA “America's Best Health Plans" 2006 report.* Care Choices offers employers a variety of traditional HMO benefit and pharmacy plans as well consumer-directed options. The plan’s network includes 40 leading hospitals and 6,200 physicians across southeast Michigan.

Preferred Choices PPO

Preferred Choices PPO is a network of 11,700 physicians, 83 hospitals and other health care providers located throughout Michigan. A network lease PPO product, Preferred Choices provides a quality network for self-funded and insured clients. Preferred Choices PPO has received full accreditation for Health Network and Credentialing Standards from URAC, a Washington, DC-based health care accrediting organization that establishes quality standards for the health care industry.

Care Choices PPO

Care Choices PPO is designed to meet the needs of self-insured employers looking for a cost-effective, one-stop solution, with access to providers across the United States. Care Choices PPO provides administration services including enrollment, billing, claims adjudication and risk management, as well as high-quality medical and pharmacy management programs. Care Choices PPO utilizes the URAC-accredited Preferred Choices PPO network in Michigan. Enrollees receive the highest level of benefits when they access Preferred Choices PPO participating providers. Care Choices PPO also provides direct access to multiple regional and national networks with single-source re-pricing of claims.
