Group Size (2-99)

As a Care Choices Qualified Agent, you can earn 5% base commission each month by writing business with new groups representing two to 99 contracts. Split contracts do not qualify - and employee and spouse are considered one contract, not two individual contracts. Commissions are paid for new business and all renewal business, and calculated against group premiums received prior to the end of the month.

Group Size (100+)

As a Care Choices Qualified Agent, you negotiate the rate of commission you will earn on a case by case basis. Commissions are paid for new business and all renewal business and calculated against group premiums received prior to the end of the month.

Care Choices reserves the right to modify or discontinue this commission program at any time.

Becoming a Qualified Agent

To become a Care Choices Qualified Agent you must have the following on file with Care Choices:

• Agent Application
• A valid copy of your Accident & Health license issued by the Michigan Office of Financial and Insurance Services (OFIS)
• A signed original copy of your Care Choices Agent Agreement
• Business tax ID or Social Security number for you or your Agency

Note: Your license must be active and in good standing by the Michigan Office of Financial and Insurance Services (OFIS).

Failure to maintain the above requirements will result in disqualification as a Care Choices Qualified Agent and you will not be eligible to earn commissions or present Care Choices to your clients.
