Get A Quote

When requesting a quote from Care Choices, please provide the current demographic information, rate ratio and benefit plan information. Please allow three to five business days for a quote to be returned to you.

Information needed to process a quote:

• Group name and location.
• Requested effective date.
• Current employee census including: gender and dependent status; age or date of birth for eligible employees; contract status of eligible employees (employee/couple/family & for four tier rate quotes - employee/child(ren) or employee/spouse, active, retiree or waive insurance).
• Name of current carrier(s).
• Description of benefit requirements.
• Date of renewal for existing plan & current rates.
• The date your response is required.
• Agent and agency name.
• Retiree rates - If a formal retiree program is offered to the entire population and you require a retiree rate, please separate that population on the census.

Preferred agents with access to the Care Choices Agent Portal can produce their own rate proposals for small grop reform employers (2-50). Agents without portal access should contact the Care Choices Business Support department. A quote request with complete information will be issued within 3-5 business days.

Groups with more than 51 eligibles should also contact the Care Choices Business Support department. Please use the Group Proposal Request Form to submit your rate quote and attach copies of census, current plan description or benefits requirements.
