Your primary care physician coordinates all your medical care. When you require specialty services, your primary care physician will coordinate and authorize care, including referrals to other specialists, ancillary providers or hospitals as necessary.

When you enroll in Care Choices HMO, you and each eligible member of your family (dependent) will select a primary care practitioner. The physician can be either a family practitioner, general practitioner, internist, or pediatrician. Each eligible family member may select a different primary care physician from the Plan's network of participating providers.

Doctors who are not accepting new patients have the symbol (o) next to their name. You may choose them only if you are already their patient.
