To search the Care Choices Provider Directory, you must first select your product.

How Do I Know Which Product to Select?

Care Choices currently offers two products. One way to be sure which product is offered to you would be to contact your employer's benefit administrator. Or, take a look at the front of your member ID card.

1. If your card has the Care Choices HMO logo on it, you would select "Care Choices HMO" from the drop-down menu above.
2. If you see Care Choices PPO printed on the front of your card, you would select " Care Choices PPO" from the drop-down menu above.
