Behavioral Health & Substance Abuse
If you think you need behavioral health or substance abuse services, you do not need to get a referral from your primary care physician. To arrange for these services, you must contact your Care Choices HMO Managed Behavioral Health Organization (MBHO). The toll-free number for this office is listed on the front of your Care Choices HMO member ID card under "Mental Health and Substance Abuse." Representatives are available 24 hours a day, seven days a week. Your MBHO will refer you to quality behavioral health providers affiliated with their network.
Note: Behavioral health and substance abuse providers listed in this online directory may not be participating with your MBHO’s network, therefore you must contact your MBHO prior to receiving services.
The professional staff at the MBHO office will talk with you, and if appropriate, arrange for your care with a Care Choices HMO behavioral health or substance abuse provider.

Care Choices HMO offers 24-hour access to behavioral health professionals who assess and coordinate needed behavioral health and substance abuse services.
