Care Choices is committed to promoting cardiac (heart) disease
prevention and management to our members.
Cardiac disease prevention is an important part of our efforts.
Care Choices can provide information and support to help you prevent
cardiac disease through lifestyle changes. Our strategy includes:
- Identification of, and education regarding, risk factors related
to heart disease
- Information on what you can do to change your risk factors
to prevent heart disease
- Promotion of healthy lifestyle changes that can help reduce
your risk for heart disease
Should you develop cardiac disease, we can help you, along with
your primary care doctor, to effectively manage it and minimize
its impact on your daily life. Our cardiac disease management strategy
includes:
- Identification of, and education to those with established
heart disease and at very high risk of developing heart disease
- Information on what you can do to reduce your risk for further
heart disease
- Promotion of healthy lifestyle modifications that can help
to reduce the effect of heart disease
