Changing Your Primary Care Physician

Please Note: This information only applies to Care Choices HMO Members.

At times there are good reasons for changing doctors. If you feel you need to make a change, you can use ISS or Customer Service can assist you. When you choose a new physician, we will ensure he/she is accepting new patients and your records will be updated accordingly.

Your change will be effective the first day of the next month following receipt of your request. Please remember to have your medical records transferred to your new physician. Call and make an appointment to meet your new primary care physician and review your medical history. Important: If you receive services from a primary care physician other than the one you chose and that Care Choices HMO has on record, you will have to pay for these services.

For assistance in choosing a new primary care physician, complete the following steps or call our Customer Service department for further assistance:

Step 1:

Get a copy of the Care Choices HMO Health Care Provider Directory.

You can search our provider directory to locate a primary care physician or you can call Member Services at (800) 852-9780 to obtain a copy of the provider directory.

Step 2:

Choose a Care Choices HMO primary care physician.

Then choose a primary care physician from the service area's list of doctors. Each covered family member can choose his or her own doctor. A primary care physician may be a:

Family Practitioner - Doctors trained in the area of family practice. These doctors care for patients of all ages and treat a wide range of illnesses.

General Practitioner - Doctors who do not limit their practice to any particular field of medicine and surgery.

Pediatrician - Doctors trained in the area of medicine for children, usually up to age 16.

Internist - Doctors who treat a wide range of illnesses for adults.
