Heart Failure Program
Heart Failure is a chronic heart condition affecting over 5 million Americans. Heart Failure occurs when the heart loses its ability to pump enough blood through the body. According to the Michigan Department of Community Health, Heart Failure is the leading cause of preventable hospitalizations and over 300,000 deaths per year. To manage and/or prevent progression of the disease, it is important to follow the disease management plan set up by your primary care physician, which may include controlling your blood pressure, taking your medications as ordered, special diet with limited fluid intake, weight management along with exercise, and quit smoking.
