Selecting a Pediatrician
Please Note: This information only applies to Care Choices HMO
Members.
Members who are age 16 or younger may choose a Care Choices
HMO family
practitioner or a participating pediatrician as
their primary care physician (PCP). Children who do not have a
pediatrician as their PCP may see a Care Choices HMO pediatrician
for general pediatric care without first seeing their PCP for a
referral.
Remember, if you want your newborn to be covered through
Care Choices HMO, you have only 30 days from the date of
your baby's birth to add him or her to your Care Choices HMO policy. Contact
Customer Service to add your baby or to discuss coverage issues.
