Getting Fit

ShapeDown®
Care Choices offers discounts to ShapeDown, a weight management for children and teens ages 6 to 18. Parents and kids work together in this innovative 10-week weight management program to learn how healthy eating, an active lifestyle and effective communication promote weight loss and family unity. A team of health professionals co-leads each class: a nutritionist, social worker, and exercise specialist. Both St. Joseph Mercy Hospital, Ann Arbor and Oakwood Hospital & Medical Center offer ShapeDown classes.

Fit Kids, Healthy Future
As a health care organization dedicated to prevention, Care Choices is concerned about the growing number of overweight children. That's why we launched the Fit Kids, Healthy Future initiative. Through a variety of partnerships, sponsorships and special events, the Fit Kids, Healthy Future initiative promotes the importance of physical fitness, good nutrition and helping children develop healthy lifestyles.

Nutritional Counselors
With a doctor’s referral, Care Choices covers up to two nutritional counseling sessions with a dietician per year for members with certain diseases or conditions. We will cover additional visits if a member needs them (they would need prior plan approval). Also, all members receive a 20% discount on professional services from nutritional counselors, with no referral required, through our Natural Options program. To receive the discount, members just need to show their member ID card at time of service.

Weight Watchers®
In a continuing effort to help fight obesity and reduce health risks, Care Choices and Weight Watchers are pleased to offer members a program to help them reach their weight management goals. When Care Choices members show their member ID card, they receive discounts on Weight Watchers Traditional, At Work, and Teen programs.

Medical Interventions

Prescription Weight Loss Medications
The Care Choices pharmacy benefit covers certain prescription weight loss drugs to help you lose weight. These medications are covered only when determined to be medically necessary for treating obesity under our guidelines. Ask your doctor if you qualify.

Prescription Drug Coverage

Bariatric Surgery
For some people who have tried other programs, their doctors may recommend bariatric surgery. This involves sealing off most of the stomach to reduce the amount of food they can eat, or rearranging the small intestine to reduce the calories the bodies can absorb. Care Choices may cover this surgery for some members, if specific criteria are met.

Need more information regarding Bariatric Surgery? Contact Customer Service at (800) 852-9780.
