Changes in Your Coverage

Care Choices HMO needs to know about any life changes that will affect your coverage, such as:

  • Marriage, divorce
  • Birth of a child
  • Adoption, change of legal guardianship
  • Death of a covered family member
  • Change of address or phone number
  • Change in a family member’s coverage eligibility

If you joined Care Choices HMO through your employer, you must report these changes to your employer. If you are an individual subscriber or are enrolled through a Conversion Plan you must report these changes within 30 calendar days directly to Care Choices HMO Customer Service to ensure that you will receive quality care and service. Conversion forms are available from Customer Service. Family members who are no longer eligible for coverage may also be eligible for conversion coverage. If you buy conversion coverage, you must pay the premium directly to Care Choices HMO. Your conversion coverage benefits may be different from your employer group plan.
