Care Choices offers those members, who have received a buyout offers from an auto company, an affordable high quality alternative to COBRA and health plan conversion coverage options. We are offering a Non-Group Option Plan that provides comprehensive coverage at a reasonable monthly premium.

To be eligible you must:

• Be an auto employee who received a buyout notice from your auto company.
• Have had the option to select Care Choices HMO as an auto employee.
• Not be eligible for a company-sponsored health plan other than COBRA.
• Not be Medicare-eligible.
