Care Choices HMO participating physicians and health care facilities can not bill you for covered services. If you believe a medical bill has been mailed to you incorrectly, please call our Member Services Department.

What are your out-of-pocket costs for health care services?

Depending on the benefit plan purchased by your employer, you will usually have some out of pocket costs for your health care benefits.

As an HMO member you may pay nominal member copay or copayments when you stay within the HMO network of participating physicians and health care facilities and follow HMO rules. You must use the Care Choices HMO health care provider network for your health care needs.

How do you determine whether your benefit plan has copays or deductibles?

Health care consumers can make sound decisions about their health care options when they are fully aware of the cost sharing arrangements their health care contracts contain. We encourage you to read your Subscriber Certificate in your Member Information Kit to learn about co-payments, limitations and exclusions.
