Diabetes Prevention & Management
Diabetes affects 18.2 million people in the United States. It
can lead to cardiovascular disease and stroke, which are leading
causes of death and serious disability.
Preventing a chronic disease such as diabetes is important. Care
Choices can provide information and support to help you prevent
diabetes through lifestyle changes. Should you develop diabetes,
we can help you, along with your doctor, to effectively
manage diabetes and minimize its impact on your daily life.
Poorly managed diabetes can result in long-term complications
such as eye disease, kidney failure, cardiovascular disease and
even amputation. We believe that following your doctor’s
recommended treatment plan is important. We also know that taking
an active role in protecting your health is key to staying healthy.

Care Choices members due for a Diabetic Dilated Retinal Exam
should complete the Diabetic
Retinopathy Evaluation form and be sure to take it with you
to your next appointment.
Be sure a copy of this form is sent to your primary care physician. Having a
record of this exam in your medical file is important for your continuity of
care. It enables your physician to track your health care needs
and provide the appropriate level of care for your condition.
