Flu Vaccine Coverage for Care Choices HMO

Care Choices HMO members may receive the flu vaccine at:

A physician’s office.

• Members may receive vaccinations at their primary care or specialist physician office.
• Members will not pay an office co-payment and Care Choices HMO will pay for the vaccine unless the member is seeing the physician for additional services.
• If the member is seen at a physician’s office for other reasons in addition to receiving these vaccines, the member will be responsible for their usual office copay.

Community Clinics staffed by the VNA Health Services, or the Visiting Nurses Association of Southeast Michigan.

• Members pay no co-payment or out-of-pocket costs at these clinics. See our list for the nearest location.

Note: Care Choices HMO does not cover FluMist™ for the 2007-2008 flu season. Members will not receive any reimbursement for receipt of FluMist™.
